Our Support at Home service offers guidance and advice to adults, including those with a physical or other disability, who are finding it hard to remain in their home or maintain their lifestyle or where a “housing related support need” has been identified. The aim of the service is to enable people to remain safe, secure and as independent as possible.
Support is provided through a team of staff managed by an area manager. Customers are consulted by staff and an agreed support plan drawn up which clearly identifies how staff will support the customer and what outcomes we are trying to achieve.
